Verification of Enrollment Request

Important Information:

• All VOE requests must be submitted through the online request form above.
• Walk-in requests will not be accepted due to required data tracking and recordkeeping procedures.
• A current and valid Proof of Residency must be on file with the school.
• Requests will be processed within two business days. 
• Completed VOEs may be picked up in the CLHS Front Office.
• A valid photo ID is required before any documents can be released.
• To be eligible for a learner's permit or driver's license, students must have maintained at least 90% attendance during the Spring 2026 semester. Students who do not meet this attendance requirement will not be eligible for a VOE, and the DPS will not issue a permit or license.

For non-DPS needs

All non-DPS requests (SSI, child support, taxes, court, etc.) must be processed through the campus Registrars Office. Please submit your request to CLHSregistrar@ccisd.net.
